Come Prepared

What to bring

New patient paperwork is completed at the office, so plan to arrive a few minutes early. Beyond that, bring:

  • A photo ID.
  • Any recent imaging, reports, or test results you already have. Helpful, not required.
  • A list of the medications and supplements you take.
  • Comfortable clothing you can move in during the exam.
  • Your questions, written down. We will answer every one.
  • A few extra minutes so we can get you checked in without rushing.
